We have an exciting full-time opportunity for a Clinical Social Worker for our Indigenous Primary Health and Wellness program.

Fraser Health is responsible for the delivery of hospital and community-based health services to over 1.8 million people in 20 diverse communities from Burnaby to Fraser Canyon on the traditional territories of the Coast Salish peoples. Build on your education and experience as you:

  • Conduct comprehensive social work and psychosocial assessments by interviewing the client and/or their family, acquiring relevant information, capturing social data and interpreting the data using a variety of tools. •Support a detailed approach to care and service delivery across the healthcare system by participating in the admission and discharge process of clients in acute care and community. •Conduct meetings to assist in developing and coordinating comprehensive care plans and community services for clients. •Facilitate timely exchange and provision of information regarding client care between the acute care facility and the community service to allow for individualized discharge planning. •Work with community service providers to ensure client care needs are met. •Arrange and participates in joint home visits to clients and/or families with other healthcare professionals. •Initiate, develops and carries out research activities and program planning and development relevant to the needs of the patient, families, hospital and community, as assigned.

To join the organization we require you to have:

  • Current full registration with the BC College of Social Workers. •Two (2) years’ recent related experience in a complex health care environment such as a community setting including one (1) year of experience working with aboriginal clients. •Knowledge of Aboriginal health approaches, protocol and cultural issues.

Equivalent combination of social work education, training and experience.
