Job description: The Sarnia-Lambton Ontario Health Team (SLOHT) is seeking an exceptional primary care leader to work effectively with SLOHT Executive Lead, Collaboration Council, working group and advisory chairs, and partners, to champion physician and primary care engagement and provide leadership to the activities of the SLOHT.

The SLOHT Physician (Primary Care Co-Design) Lead (the Lead) will support primary care to co-lead the re-design and implementation of a true integrated care model to increase access to care in the community. They will work in partnership with the Primary and Specialist Care Advisory Council (PSCAC) to engage and represent the perspectives of clinicians during the planning and implementation of co-design initiatives that improve integrated care. The Lead will work with other primary care stakeholders to ensure meaningful engagement, communication, and coordination with the work of the OHT.

Qualifications:

  • System thinker with understanding of clinical leadership in health system integration, design, planning and implementation
  • Focused on ensuring patient, family and care partners have a central role in design, planning and decision making
  • Excellent communicator, strategic thinker, and team player with a passion for health system improvement
  • Self-directed and highly motivated; able to handle and navigate clinical and operational ambiguity
  • Ability to influence, achieve consensus and drive change

Skill, Ability and Experience:

  • A practicing primary care physician holding a valid certificate of registration and in good standing with the College of Physicians and Surgeons of Ontario
  • Actively practicing within Sarnia-Lambton
  • Established positive relationships with PSCAC and peers
  • Expertise, understanding, and/or passion for population health management, quality and process improvement, and digital health
  • Ability to manage competing demands and meet tight timelines
  • Knowledge of provincial health care structures, processes, funding mechanisms and current priorities is strongly preferred
  • Formal leadership training, change management, and experience in academic detailing or mentoring considered an asset

Principal Duties:

The SLOHT is a committed team of health and social service providers and community members who are dedicated to better connecting health care across Sarnia-Lambton as part of the system transformation currently underway in Ontario’s healthcare system. At maturity, OHTs will be clinically and fiscally accountable for delivering a full and coordinated continuum of care for our entire attributed population of over 118,000 people. Our key objective is to improve patient/client, care partner, and provider experience and patient/client whole health outcomes equitably, and cost-effectively. We apply a health equity, quality improvement, population health, and co-design approach to everything we do. To better serve everyone, we need to think differently.

The Lead will provide clinical leadership aligned with the OHT’s purpose, priorities, and values. This individual will actively work to improve engagement, participation and system co-design amongst primary care providers in Sarnia-Lambton. This individual will contribute to advancing the work of the SLOHT by working closely with the Executive Lead and Collaboration Council in leading and/or participating in prioritized OHT meetings and system design sessions. Additionally, this Lead will represent the SLOHT locally, regionally, and provincially. Where possible, to provide some predictability in scheduling, we will attempt to schedule local meetings requiring clinical input against the clinical leads clinic schedule.

Compensation based on SLOHT physician leadership rates. Time allocation estimated to be equivalent to one day per week with additional opportunities for involvement possible based on need and interest.
