Job description: Your Opportunity:

As a Community Health Representative (CHR), your goal is to help Indigenous clients feel comfortable with the health services they receive at both the area Health Center and in their community. You are responsible for case management, patient advocacy, referrals, health literacy support, and logistical support for discharge planning. With compassion and understanding, you support your clients at medical appointments and case conferences, and help them navigate services between AHS, Indigenous Services Canada, and community funded programs. On occasion, you may assist with language interpretation and the education of healthcare teams to ensure culturally sensitive care (e.g. smudging, Elder services). As part of the Health Services team, you will serve clients in the Peerless Health Care Center, The Red Earth Creek Community Health Services Center and the Community of Peerless Trout First Nation. In addition to a competitive rate of pay, AHS has a Remote Retention Allowance in place to an annual maximum of $3,000. This allowance is non-pensionable and is payable on an hourly basis for all hours paid at the basic rate of pay. As the Community Health Representative (CHR), you will advocate for and support Indigenous clients and families within health care facilities and within the community setting. You will promote awareness of AHS programs and services as well as external resources such as FCSS, Children’s Services, and local social service agencies. You will provide consultation to the health care team regarding culturally appropriate service and create education programs for both AHS staff and Indigenous clients. You will develop meaningful partnerships between AHS and Indigenous communities and encourage Indigenous students to learn about Health Care and start their career at AHS.

As a Community Health Representative, you will work in partnership with community health staff in the provision of screening programs, health education and communicable disease control, and enhance the community capacity through strong community partnerships.

Required Qualifications:

Completion of Grade 12. Additional Required Qualifications:

High degree of cultural competence as it relates to working with Indigenous people. Ability to understand and speak the Cree language. Flexibility to travel and work occasional evenings and weekends as needed. As a bona fide occupational requirement, and under Section 11 of the Alberta Human Rights Act, only Indigenous applicants will be considered. We encourage Indigenous candidates to self-identify. Ability to communicate effectively with internal and external partners in a respectful and collaborative manner. Proven ability to foster partnerships, and establish and maintain strong working relationships with First Nations and Metis communities, public agencies and government. Working knowledge of computer applications including basic word processing, email, videoconferencing, and internet access and data entry required. Preferred Qualifications:

College certificate/diploma in related health care discipline, such as Community Health Representative, Social Work, Nursing preferred (If part of a registered profession, registration in good standing with applicable governing body). Demonstrated ability and comfort with coaching and teaching individuals and groups. Ability to solve routine and standard problems. Evidence of initiative, flexibility and strong commitment. Demonstrated punctuality, adaptability, discretion and confidentiality.
