What we are looking for:
As a member of the Adult Diabetes Education Clinic team, you will provide diabetes education to our patients in the outpatient clinics in both group and individual settings. In this role, you will, in collaboration with the interdisciplinary health care team, be responsible for comprehensive assessment, care planning and nutritional education for patients and families with diabetes. You will practice in an environment where the emphasis is on a high standard of diabetes care, professional practice and patient experience.
You will co-ordinate your services with the other staff in the Clinic, which will require day and evening as well as potential travel to our satellite sites. In addition, you will participate in ongoing quality improvement and promotional activities, committees and the development of standards and policies as it relates to our Diabetes Program.
Your strong diabetes knowledge and counselling skills, coupled with well-developed analytical, problem-solving and interpersonal skills, will assist you in the provision of quality patient care.
What you bring to the role:
- Current registration with the College of Dietitians of Ontario in good standing
- Membership in the Dietitians of Canada preferred
- Certified Diabetes Educator designation required (or demonstrates strong knowledge in diabetes education & management and willingness to obtain CDE certification)
- Bilingualism in a South Asian dialect is an asset.
- Certified Pump Trainer is an asset.
- Evidence of ongoing professional development, continuing education and leadership skills
- Excellent interpersonal, communication and organizational skills
- Demonstrated proficiency in diabetes pharmacotherapy management and education – including insulin and OHA adjustment
- Computer skills are essential
- Ability to work independently and as part of an interprofessional health care team
- Candidates must have own transportation and the flexibility to travel between Markham, Uxbridge, and satellite clinics, as necessary
- Demonstrated good attendance and performance records with the ability to maintain these same standards
- Demonstrates a commitment to a healthy and safe workplace for self and others (staff, patients, families, etc.) by taking all reasonable precautions and working in compliance with hospital related policies, health and safety legislation and best practices and completing relevant mandatory education as required
Work Schedule:
The successful candidate will need to be available to work rotating shifts Monday- Sunday.

Compensation:
BAND M: $34.13 – $45.86 per hour
