Position Summary
If you have great communication skills and the ability multitask, we have a wonderful career opportunity for you as a Home Health Nurse.

As the Home Health nurse, you will support the client’s achievement of safe realistic and reasonable goals by assessing, planning, coordinating, organizing, implementing, evaluating and transitioning nursing and other care services for the client. Working in collaboration with the client, family and other care providers you will develop, implement and adapt a care plan, deliver direct care, assess service eligibility and provide care management to help clients manage their own care and navigate through the various services available within the community setting.

Duties may include, but are not limited to, case management, wound care, palliative care and general nursing assessment and intervention. The population is varied but primarily older adults with multiple comorbidities. Use of computer programs such as Meditech and Goldcare will be required.

The successful candidate will be a critical thinker, strong communicator, advocate and able to work independently and in a multidisciplinary professional environment.

This is a Casual opportunity. Casual means there are no guaranteed hours and work requirements may vary from 0 – 37.5 hours per week. Casual employment includes: short-notice, on-call availability; workload coverage during a temporary absence; and/or providing relief coverage in a specific position. Once hired into a casual role, employees are eligible to apply for internal postings including permanent part-time or full time positions. Shifts are rotating days, shift times are 08:30 to 16:30.

Qualifications
Graduation from an approved school of nursing with current practicing registration with the British Columbia College of Nurses and Midwives (BCCNM).

Candidates must also hold a valid B.C. driver’s license.
