What you’ll do

  • Assess and identify patients’ and families’ actual potential strengths and limitations using physiological, psychological, social-cultural and spiritual needs by performing duties such as completing a nursing history and performing a head to toe assessment.
  • Contribute to the development and modification of individualized nursing care plans in collaboration with the RN and input from the family by performing duties such as formulating actual and potential patient problem lists, identifying short and long term goals, providing input to the RN regarding prioritizing care, identifying nursing interventions, and evaluating and monitoring patient and family responses to nursing interventions.
  • Deliver and collaborate in planning and coordinating the nursing care of patients by documenting and updating information about the patient’s condition, reporting problems and/or potential problems to an RN and/or physician and changing the patient’s condition as required, and completing admission, teaching, discharge and transfer sheets.
  • Review and interpret the established nursing plan of care, provides nursing care and assists patients with hygiene, grooming, toileting needs, ambulation and mobility, and feeding, and perform procedures such as dressing changes and medication administration to assigned patients according to established policies and procedures.
  • Follow diagnostic treatment plans including collecting specimens and preparing and caring for patients undergoing diagnostic tests, examinations, or procedures.
  • Participate in and supports patient and family teaching to patient care and discharge planning. Participate in the orientation of new LPN staff and LPN students, and assist in the orientation of other members of the health care team.
  • Perform other related duties as assigned.

What you bring

Qualifications

  • Graduation from a recognized program for Practical Nurses or an equivalent combination of education, training and experience. Current full practicing licensure with the British Columbia College of Nurses and Midwives (BCCNM).

  • Ability to operate related equipment.

  • Knowledge of social, economic, political and historical realities impacting indigenous communities and familiarity with Indigenous Cultural Safety and anti-racism and accompanying reports (BC DRIPA, TRC, etc.).
