JOB PURPOSE:

Reporting directly to the Community Health Needs Assessment Research Lead(s), the Community Health Needs Assessment (CHNA) Coordinator will work as part of a CHNA Engagement Team within the department of Population Health. The CHNA Engagement team consists of two Research Leads, Community Developers, and the CHNA Coordinator. The CHNA Coordinator will assist in the planning and facilitation of Horizon’s CHNA process which includes coordinating the CHNA communication strategy, and supporting the coordination of the planning, assessment, and accountability, and evaluation aspects of the CHNA process. Twenty-one community clusters, defined by the New Brunswick Health Council, that sit within Horizon’s region are engaged through the process of a CHNA. The CHNA Engagement Team will be engaging directly with two to three separate communities at any given time and the CHNA Coordinator will coordinate the communication plan across all communities as required throughout the entire CHNA cycle.

For this work, a CHNA is defined as a community engagement process facilitated by Horizon with community partners to identify and prioritize population health needs in local communities serviced by Horizon with the following objectives:

1) Enable Regional Health Authorities in the work of determining health service need and prioritizing health service delivery.

(2) Enable the Department of Health in the work of health care system planning and cross-department collaboration.

(3) Strengthen relationships between RHAs and local stakeholders involved in the work of improving population health in place-based communities.

QUALIFICATIONS:

  • Coordination of the CHNA process to ensure alignment across the project
  • Track CHNA expenses and timelines
  • Communicate/engage with community partners and CHNA stakeholders.
  • Coordinate and track CHNA facilitation across communities
  • Schedule and organize meetings and connections
  • Prepare communication and meeting materials (e.g. infographs, bulletins, etc.)
  • Support group facilitation
  • Develop PowerPoint presentations
  • Manage databases
  • Edit final CHNA reports
  • Provide support to the Research Lead
  • Support the implementation and execution of a CHNA evaluation process
  • Work collaboratively with other Department of Population Health team members
  • Collaborate with Horizon’s Communications Department and coordinate the translation of CHNA documents
  • Ability to attend work on a regular basis.
  • Ability to perform the duties of the position.
  • Good work record.
  • Written and spoken competence in English (B2.1 or Advanced) and spoken competence in French (B1.2 or Intermediate Plus) are required.

Other Essential Requirements/Qualifications:

Some travel will be required and is eligible to work from home.

KEY RESPONSIBILITIES:

  • University degree in the health sciences, or social sciences and/or relevant project coordinating experience (Any equivalent combination of education and experience will be considered)
  • Interest in population health and health equity
  • Project management experience and training
  • Ability to manage complex relationships
  • Excellent interpersonal, oral, and written communication skills, both with internal systems and the public
  • Strong computer skills such as Microsoft suite, database management, survey platforms, etc.
  • Ability to work independently and with minimal supervision
  • Demonstrates a proven track record in managing complex projects
  • Written and spoken competence in English and French required.
  • Ability to work evenings and weekends as needed
  • Ability to travel throughout the region
  • A valid driver’s license and the use of a vehicle
  • Good work and attendance record
  • Physically able to perform assigned duties.
