Position Overview
The Clinical Facilitator works as a member of the Health Outreach and Community Support (HOCS) team to respond to requests to assist with the health and social service needs of marginalized, at-risk populations and the agencies that serve them. The HOCS service is population focused and a regional resource to all areas where population needs surface. This includes the emergency homeless shelters, Housing First programs, other partner agencies, regional health and social service sites as well as acute care sites.
The Clinical Facilitator provides outreach consultation, coordinated service planning assistance, capacity building support, and nursing assessment and services. The Clinical Facilitator works towards increasing the individual capacity of the HOCS population and increased organizational capacity of the sector agencies to provide services to the population. The Clinical Facilitator works in partnership with agencies, Primary Care, Home Care and Home Care Nursing to bridge service gaps and coordinate continuity of care in the community. Clinical practice is provided to meet immediate needs, support engagement, and establish person-centred plans while connections to longer term services are coordinated.
The Clinical Facilitator adheres to the documentation, consent and privacy standards of the Winnipeg Regional Health Authority. The Clinical Facilitator ensures that s/he performs her/his duties in a manner which is consistent with the standards of the WRHA and the College of Registered Nurses of Manitoba.
The Clinical Facilitator responds to the direction and supervision of the Initiatives Leader. The Clinical Facilitator is a direct contact for community agency staff as well as health and social service staff to meet individual health needs and facilitate service access. The Initiatives Leader is the direct contact for program management issues with management staff of community agencies, WRHA, and government.
Experience
- Minimum three years community nursing with experience coordinating continuity of care.
- Minimum three years related community experience working with marginalized populations, including individuals experiencing homelessness as well as addictions and/or severe and persistent psychiatric disabilities.
- Demonstrated experience with independent decision-making.
- Experience with developing person-centered service/care plans.
- Experience with health and social service systems including knowledge of available resources for referral.
- Experience with providing capacity building support.
- Experience and compliance with protocols for consent, practice, documentation, and privacy.
Education (Degree/Diploma/Certificate)
- Nursing (RN/BN required, BN preferred).
- ASIST Certificate (Applied Suicide Intervention Skills Training).
Certification/Licensure/Registration
- College of Registered Nurses of Manitoba member in good standing.
Active participation in professional associations e.g. Association of Registered Nurses of Manitoba (ARNM) preferred. * Valid driver‘s license and vehicle.

Scope of practice as documented in the Regulated Health Professions Act (RHPA). * Computer literacy for communication, word processing and utilized clinical applications.
Physical Requirements
