This pharmacist position will provide clinical services in the Emergency Department at the RAH. Clinical duties may include medication reconciliation, developing and implementing care plans, seamless care activities, opiate medication management, and antimicrobial stewardship activities for patients in the ED. The pharmacist will collaborate with a multidisciplinary team. Clinical placement is subject to change based on operational needs.

Description:

As a Pharmacist I, you will be responsible for the safe, effective and efficient preparation and distribution of pharmaceutical services. You will manage medication therapy for patients, both initially and on an ongoing basis. In addition, you will provide assessment, intervention and resolution of potential or actual drug-related problems and oversee activities related to compounding and packaging.

Required Qualifications:

Bachelor of Science in Pharmacy. Active or eligible for registration with the Alberta College of Pharmacy (ACP). Additional Required Qualifications:

Experience with medication reconciliation is required. Additional Prescribing Authorization is required (or must be achieved within 1 year of the date of hire). Preferred Qualifications:

Experience working in an emergency department (ED) is preferred. Experience at the RAH preferred. Experience working in a collaborative model is preferred. Experience working with vulnerable populations preferred. Competence with Connect Care is preferred. Completion of a Canadian-accredited pharmacy residency program is preferred. Advanced clinical pharmacy degree, e.g., post-baccalaureate PharmD preferred.
