Position Overview

Monitor the provision of resident care by the nursing team on an assigned unit, acting as the team leader. Assess the resident’s needs in accordance with the RAI-MDS tool and provide a plan of care and interventions based on an evaluation of outcomes.

Duties and Responsibilities

Administration of Medications and Treatments:

  • Administer and document medications and treatments for all residents, and assess effectiveness
  • Report resident response to medication and treatments to physician/nurse practitioner
  • Transcribe and process all orders received

Communication and Documentation:

  • Chart care provided, behaviours and incidents
  • Respond to resident and family inquiries
  • Educate residents, families, and staff
  • Complete RAI-MDS assessments, RAPs and care plans according to schedule
  • Act as a mentor for students during clinical placement
  • Provide guidance and direction to Personal Support Workers, and other staff working on the assigned unit

Resident Care:

  • Assist as necessary with resident care and transferring
  • Ensure resident and family are comfortable during palliative phase
  • Monitor levels of liquid oxygen in portable ranks and fill as necessary

General:

  • Incumbents must keep up-to-date on all relevant legislation, collective bargaining agreements, County Policies and Procedures, etc. ensuring that these are implemented as prescribed
  • Incumbents must possess the physical ability to meet the duties and responsibilities of the job description and/or the requirements identified within the position’s Physical Demands Analysis

Qualifications

Minimum Formal Education:

  • Registered Practical Nurse Diploma
  • Registration with the College of Nurses of Ontario
