Reporting to the Team Lead/Manager of Awasisak Indigenous Health Department, The Indigenous RN Case Manager is responsible for providing advanced patient liaison services including discharge, follow-up, and transitional planning. In your role as RN you will provide support for children and their families needing services in the community and support transitioning from hospital to community. You will be working within a patient family centred approach and will be responsible for assessing, planning, implementing, and evaluating patient care in conjunction with the interdisciplinary and community teams. Responsibilities are working within the Awasisak and multidisciplinary teams to ensure Indigenous children and their families from Central to Northern Alberta, BC, SK and Manitoba. Have transitional discharge planning that is timely and appropriate. Cultural needs are recognized, supported and integrated in the care. Multi-action plan – to implement communication strategies for the child’s health care providers know who is involved. Provide emotional, mental, spiritual and practical support to children and their parents while children are within the Stollery Children’s Hospital as an Inpatient or Outpatient. The position provides a range of resource-based services and interventions relevant to the child, family and Awasisak Indigenous Team. For example: To take on a case load of pediatric patients and their families and provide discharge planning and teaching to successfully transition back to community and organize and support follow up appointments with sub-specialist and pediatricians. The incumbent will assist the Awasisak Indigenous Team with implementation and coordination of resourcing tasks, and be familiar with the healthcare, community and government services and resources that can provide support to patients and families.

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS.

Required Qualifications:
Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College and Association of Registered Nurses of Alberta (CARNA). Current Basic Cardiac Life Support – Health Care Provider (BCLS-HCP). Additional Required Qualifications:
High degree of cultural competence embedded experience as it relates to working with Indigenous people. At least 5 years of experience providing significant direct health care services to Indigenous families/communities; or being someone who has lived at minimum 5 consecutive years as a participant member within an Indigenous community. Case management coordination i.e. discharge planning, teaching medical complex care scenarios to patient and families. Minimum requirements. 2 years minimum required of hospital in patient experience. 2 years charge nurse or leadership equivalent experience. 2 years pediatric health care experience, or mental health experience. Preferred Qualifications:
Home care experience preferred with knowledge of transitional planning. Indigenous programming or health care certificates. Indigenous heritage an asset. Fluency in an Indigenous language is an asset (preferably Cree or Dene). We encourage Indigenous candidates to self- identify. Knowledge of local Indigenous history and experience in local Indigenous culture. Broad knowledge of Indigenous history and cultures within Alberta.
