Job description: Your Opportunity:
Working under the general supervision and guidance of the Supervisor, Health Information/Records Management, this position provides operational administrative support for the Records Management section of Health Information Management. Within a team environment the incumbent is responsible for performing activities which directly support the security, maintenance and integrity of the patient record through the life cycle regardless of medium (electronic and/or paper). Primary responsibility is to provide frontline reception and is the first point of contact for staff, physicians and stakeholders. Receives all discharge records and performs reconciliation process. Performs verification on all documents (paper/electronic) prior to filing to ensure integrity of the patient record. Responsible for Verification/Validation and deficiency management activities. Follows process for Physician Curtailment and liaises with Physician Secretaries, Site Medical Office and Medical Affairs with respect to distribution of weekly reminder letters. Provides assistance and information to physicians regarding documented chart deficiencies; monitors the physician dictation area/boxes and provides follow up of transcription issues/corrections. Responsible for attaching transcribed reports to patient records. Retrieval of charts for Medical Review Board and filing of patient records to permanent file. Responsible for daily break coverage for Registration, Switchboard and Patient Information with coverage for unscheduled absences, as required. Responsible for chart moves and Iron Mountain activities. Will support Connect Care activities for Launch 6 education and training involving e-learning modules, instructor-led training, end user training, practice in the PLY environment and as the Super User at “Go Live”. Ensures workload is current and coverage is maintained. Other duties as assigned.

Required Qualifications:
Completion of Grade 12 or equivalent. Additional Required Qualifications:
Medical Terminology Certificate required with testing prior to interview (60% pass). Typing speed of 40 wpm required with testing prior to interview. Strong computer skills including proficiency with Microsoft Office programs (Word, Excel, and Outlook) required. Ability to maintain confidential information. Must be able to set and organize own work priorities and adapt to change. Must be able to work concurrently on a variety of tasks/projects in an environment that may be stressful with individuals having diverse personalities and work styles. Team player with demonstrated ability to work within a team environment, modeling behaviors that support and contribute positively to team operations and working relationships. Ability to work independently with minimal supervision. Must be physically able to perform duties involving standing, sitting, bending, reaching and stretching for extended periods of time. Must be able to lift/push/carry up to 50 lbs. and be able to push and pull charts out of mobile shelving. Performs all functions with a high degree of accuracy and maintains or exceeds established productivity standards. An excellent attendance record, reliability and personal suitability must be demonstrated through employment, interview and references. Equivalent combination of education and experience may be considered. Applicants selected for an interview will be required to take the typing and medical terminology tests. Preferred Qualifications:
Post-secondary education applicable to position – MOA/Unit Clerk/Admitting Clerk preferred. Two years related experience in a hospital or medical office setting preferred. Experience with computer applications common to Health Records (e.g. PCIS, Netcare, PowerTrac, PowerMonitor, eSignature, eMon, other ADT systems, Connect Care) is preferred. Experience working with a straight or consecutive numeric filing system is preferred. Knowledge off site storage activities is preferred.
