Job title: Bilingual Diabetes Education Program Assistant

Job description: Our rapidly growing team is in need of a Bilingual Diabetes Education Program Assistant supporting the Diabetes Education Program (DEP) team and the DEP Managers in managing daily operations. This is a 12-month, temporary full-time position with the possibility to extend. This position is primarily remote with potential for in clinic support as site needs change. Access to a vehicle and a valid driver’s license is an asset as you may be required to travel between sites occasionally.

Responsibilities:

  • Booking and confirming patient appointments
  • Contact patients who are “lost to follow up”
  • Following up with patient inquiries to ensure they are receiving excellent care
  • Assist with scheduling of DEP workshops and appointments
  • Support delivery of virtual workshops through Acuity and Zoom platform
  • Prepare patients for their individual appointment with the diabetes educator by downloading necessary devices and preparing patient information.
  • Assist with entering relevant data into the patient chart
  • Patient interaction on the phone or in the clinics
  • Complete patient forms for insurance, government programs and financial assistance programs
  • Assist with recruitment, enrollment and data management for Diabetes Education Projects, by enrolling patients in our programs
  • Performing special projects as assigned
  • Participate and assist in the planning of diabetes community events
  • Promote the Diabetes Education Program to patients and help create efficiencies within the team.

Requirements:

  • University or college degree in health services preferred.
  • Excellent customer service skills
  • Experience with Diabetes is an asset, but not mandatory
  • 1-2 years of experience in a customer facing role.
  • Ability to travel to multiple sites.
  • Ability to work flexible hours, including some evenings.
  • Able to communicate effectively with patients from diverse backgrounds in French and English.
  • Medical terminology knowledge would be considered an asset.
  • Advanced MS Office computer skills.
  • Ability to communicate with patients via telephone
  • Able to work in a fast-paced changing environment.
