Job description: Summary Reporting to the Program Coordinator and under the direction of the Nurse, Occupational Therapist and Social Worker, this position works with the interdisciplinary tertiary mental health team to provide a variety of day-to-day physical, emotional, and social supports, advocacy, life skills assistance and support to access resources to individuals with severe and persistent mental illness. Works collaboratively with the interdisciplinary team in the development of individualized rehabilitative treatment plans. Contributes to the overall evaluation of the client; assesses potential emergency situations and makes necessary interventions as required; and establishes, maintains and enhances therapeutic relationships based on respect. Performs clinical procedures such as collecting specimens and weighing clients. Promotes independence for client while supporting them with activities of daily living such as personal care, including bathing, grooming, dressing, oral hygiene, toileting needs, perineal and continence care; community appointments/errands/activities; meal planning; and preparation. Participates in team meetings, engages in team planning and quality improvement activities, and maintains and updates own knowledge.

Education Grade 12, Social Services Diploma or Certificate from a recognized Health Care Worker educational program, such as (Health Care Support Worker Certificate or Certificate in Community Mental Health), plus two (2) years’ experience working in a psychiatric setting with individuals who have complex mental health and addictions issues, or other qualifications determined to be reasonable and relevant to the level of work.
Valid Class 4 vehicle license (unrestricted) as required. Local area travel may require the use of a personal vehicle for which mileage reimbursement will be paid.
Certificates in Cardiopulmonary resuscitation (CPR) – Basic Cardiac Life Support (BCLS) Level, First Aid and Food Safe.
