Reporting to the Manager, Nutrition and Food Services you will work as an active member of our interdisciplinary team. You will be responsible for providing quality nutritional care and education to patients with Diabetes, their families and/or their significant others within the hospital. In addition, you will be required to contribute to program planning, implementation and evaluation in an interdisciplinary team concept. You will practice according to the current Standards of Practice of the College of Dietitians of Ontario, and the Policies and Procedures of SHN.

Responsibilities:

  • Providing a specialty-based focus, functioning in a Registered Dietitian role demonstrating a high level of autonomy and expert skill to formulate clinical decisions and appropriately care for adult clients with Diabetes
  • Conducting screenings and client assessments to identify the need for nutritional interventions
  • Providing education and counseling for patients with Diabetes and their families, including assisting the client in gaining knowledge, skills and confidence in self-management
  • Acting as a resource to members of the inter-professional health care team
  • Completing required documentation, including client charting, workload statistics, and quality assurance activities
  • Attending clinics, client care rounds and educational sessions
  • Participating in the design, implementation and evaluation of continuous quality initiatives

Requirements:

  • Member of the College of Dietitians of Ontario (CDO)
  • Certified Diabetes Educator
  • 1-2 years′ experience working in Diabetes
  • Demonstrated knowledge of clinical/nutrition guidelines appropriate for clients with Diabetes and ability to recommend treatment protocols
  • Demonstrated knowledge of adult learning principles and facilitation skills
  • Demonstrated ability to plan programs and develop educational materials to support nutrition education
  • Demonstrated exceptional interpersonal, verbal and written communication skills and judgement when relating to clients, families, co-workers and members of the interdisciplinary team
  • Demonstrated ability to work effectively within an inter-professional team and independently
  • Demonstrated ability to build and maintain effective relationships with a diverse range of people
  • Demonstrated ability to organize workload and establish priorities effectively
  • Demonstrated problem solving skills and ability to exercise independent judgement and to assess situations and problems efficiently and effectively
  • Motivated self-directed learner, who takes initiative and seeks learning opportunities to enhance client care and professional competence
  • Demonstrated commitment to the philosophy of patient centered care
  • Demonstrates values consistent with SHN’s Code of Conduct and is committed to fostering a healthy and positive work environment in accordance with Health & Safety policy and legislation
  • Utilize protective equipment provided and identify and reports hazards
  • Engages in ethical conduct and strives to understand and appreciate the diversity of our patient/staff population and community
  • Excellent work performance and attendance

Asset:

  • Membership with Dietitians of Canada
  • Experience working with people from various ethno-cultural backgrounds
  • Knowledge of Meditech computer system, Excel and internet
  • Ability to speak another language
