This is accomplished across four geographic management zones which are responsible for the operation of acute care health centres and the provision of a variety of inpatient, outpatient services including academic, tertiary, quaternary care and community based programs and services including continuing care, primary health care, public health, and mental health and addictions.
Responsibilities
Reporting to the Manager of Patient Flow and Medical Social Work, the Health Care Social Worker will provide professional social work services to patients and their families. Services may also include the provision of support groups. With considerable latitude and encouragement to initiate and work independently, the incumbent will be an adaptable and flexible person with the ability to work collaboratively in a variety of work settings. Social Workers are expected to exhibit sound professional judgement and initiative while working within the framework and objectives of Nova Scotia Health. Some home visits and travel are required.
Qualifications
- Master’s Degree in Social Work
- Registered or eligible for registration with the Nova Scotia College of Social Workers (NSCSW)
- Minimum of two (2) years clinical experience in an acute care medical/surgical setting
- Where there are no successful applicants who meet the above noted qualifications, consideration may be given to applicants with a Master’s of Social Work who don’t have the required experience, or to applicants who have a Bachelor’s of Social worker and are enrolled in a Master’s program who are expected to graduate within the year
- Evidence of extensive knowledge of community resources
- Evidence of ability to work independently with a multidisciplinary team as well as with both hospital and community based services/agencies
- Evidence of ability to coordinate/develop/document and implement care plan for hospital discharge and safe transition back to the community or institutional setting
- Demonstrated use of innovative problem solving and application of evidence based practice to patient services
- Knowledge of Joint Discharge Operations Committee and its purpose
- Valid driver’s license and access to reliable transportation
- Physical ability to perform the duties of the position
- Good attendance and work record
- Competencies in other languages an asset, French preferred

Hours of Work
- Full Time, Long Assignment; 75 hours bi-weekly

Salary Information
$34.9873 – $45.9942 hourly
