JOB PURPOSE:

The Regional Director of Population Health will be the key driver of creating and leading a team focused on improving quality of life for each member of the community. Integrate business and financial expertise with clinical practice and quality data to improve the health of the region’s individuals, families and community.

Mentoring, coaching and supervising teams in developing and executing practical solutions and innovative methods and solutions to improve quality and health outcomes will be a primary goal in this role.

Report to the Vice President of Community and be accountable for designing and implementing the organization’s population health strategy, providing outreach to providers, partners and stakeholders.

Successful candidate will be a high performer with organizational proficiency as well as dependable. He/she will have an innovative, learning mindset.

QUALIFICATIONS:

  • Master’s degree with a minimum 3 years’ experience or a Bachelor’s degree Healthcare Administration or related field.
  • Minimum 5 years’ experience in project and/or program management and/or Population Health
  • Leadership, management experience and complex organizational management skills preferred.
  • Managed care experience preferred
  • Professional work style with high level of problem-solving skills
  • Transformational change leader and systems thinker with a demonstrated track record of leading change in a complex environment.
  • Ability to attend work on a regular basis.
  • Ability to perform the duties of the position.
  • Good work record.
  • Written and spoken competence in English is required.

Other Essential Requirements/Qualifications:

  • Willing and able to travel throughout the province. Must have a valid NB driver’s license.

KEY RESPONSIBILITIES:

  • Responsible for the operations of the Population Health team. This includes managing the design, development, and implementation of projects, ensuring accurate, timely, and useful data are provided to Horizon to guide decisions for facility/department/function strategies.
  • Establishing and implementing innovative processes, tools, and best practices, and for building and managing a team of highly capable professionals.
  • Coordinating with other data community and acute leaders to ensure cooperation and consistency on enterprise initiatives. They have a notable presence in the community, including user groups and forums.
  • Manage analysis supporting strategic management decision making. Communicate and coordinate effectively with stakeholders to understand strategic priorities, provide support for those priorities, and ensure decisions are made based on sound analysis and data. Develop appropriate relationships with stakeholders and partners such that incumbent is viewed as a colleague and problem solver.
  • Consult on strategic initiatives. Work with key organizational stakeholders to provide support on system-wide strategies. Coordinate with other healthcare and community organizations to share knowledge and engage in other collaborative arrangements.
  • Consult on improvement initiatives. Partner with financial operational and clinical leaders to identify and improvement opportunities, and work with them to measure and realize improvements for the population.
  • Responsible for building and training an effective team, including employee engagement, adoption of best practices, and problem resolution. Review, prioritize, and assign work for staff. Coach and develop supervisors and staff.
  • Develop and oversee monitoring and reporting procedures to ensure deliverables and budget are on track. Communicate program status and areas of risk or opportunity.
  • Serve as a liaison between the corporate and local/regional entities to ensure local needs are met while complying with regional practices and standards.
  • Work closely with cross-functional leaders to clarify program goals and create operational plans to ensure successful execution of the relationship with our stakeholders and partners
  • Accountable to propose department budget and manage daily operations within the parameters of the budget. Provide day-to-day leadership of direct reports, cross-functional committees. Work collaboratively with teams to plan, design, develop, implement, and manage programs to ensure ongoing performance expectations for the program are met.
  • Manage stakeholder and community relationships and expectations.
  • Handle all management level responsibilities for direct staff, including performance reviews, employee development, hiring, coaching, counseling, and retention.
  • Partner with cross-functional leaders, provider groups, and vendors to lead organizational change.
