Position Summary
The Pharmacist provides comprehensive pharmacy services to the Total Parenteral Nutrition program, with additional coverage to other inpatient and outpatient areas at the Toronto General site, as needed.
Duties
- Monitoring adequacy of nutritional care while identifying and preventing potential problems and complications
- Operating under a medical directive to prescribe the micronutrient, trace element, vitamin and medication additives within the PN formulation based on an assessment of the patient’s nutritional requirements, medical conditions, clinical history and concomitant medication
- Operating within a medical directive to initiate blood work to enable monitoring of PN according to UHN policy
- Identifying, resolving and preventing drug therapy problems with the patient and health care team
- Following-up on medication incidents and promoting safe medication practices
- Responding to drug information questions
- Participating in quality assurance activities related to PN and providing patient education
- Conducting best possible medication history (BPMH) interviews, performing medication reconciliation, reviewing and dispensing all doctors’ medication orders for appropriateness
- Monitoring drug therapy for effectiveness, toxicity and adherence to evidence based medicine/guidelines
- Facilitating hospital discharge
- Providing direct and/or functional supervision of technician staff
- Facilitate and support clinical teaching and research activities within the department, while exemplifying UHN’s values of caring, excellence, teamwork, innovation, integrity, leadership, and respect
Qualifications
- Possess a Bachelor’s of Science Degree in Pharmacy, Doctor of Pharmacy degree or equivalent
- Certificate of competency from the Pharmacy Examination Board of Canada
- Active registration with the Ontario College of Pharmacists (Part A)
- Hospital pharmacy practice experience with patient-centered care experience is a requirement
- Minimum of 2-5 years experience in an inpatient acute care setting is a requirement
- Experience in total parenteral nutrition is an asset
- Experience in a critical care setting is an asset
- Hospital Pharmacy residency an asset
- Membership in the Canadian Society of Hospital Pharmacists and/or other professional societies an asset
- Excellent verbal and written communication skills
- Excellent customer service and interpersonal skills
- Excellent organizational and time management skills
