Then this position with the Central Zone Continuing Care Access Centre virtual team may be the job for you! The successful candidate can work from any Alberta Health Services site in Central Zone, and that space can be negotiated. Reporting to the Continuing Care Access Centre Manager, the Registered Nurse – Continuing Care Access Centre Intake Nurse will join an amazing team of nurses that focus on intake and screening. You will be the initial point of contact for clients who may require Home Care services, Designated Supportive Living or Long-Term Care Services. You will be responsible for identifying client needs and directing clients to appropriate resources (internal and external). You will work in multiple computer programs while completing clients’ initial phone assessments in real time. You will conduct the initial contact assessment and liaise with relevant home care case managers. You will be able to effectively prioritize referrals from multiple sources and work well independently. You will workdays (0815-1630 or 0915-1730), evenings (1215-2030) and weekends (0815-1630) as required.

Description:

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit Holders shall be paid $35.87/hour – $43.82/hour. Upon being entered into the register of Registered Nurses, the rate of pay shall be adjusted to the rate of pay applicable to a Registered Nurse in accordance with AHS/UNA Article 25.02(b)(i) or (ii) as applicable. Required Qualifications:

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support – Health Care Provider (BCLS-HCP). If you have not practiced as a Registered Nurse within the last five (5) years, completion of the following free courses is required: CRNA Medication Administration Certificate (https://connect.nurses.ab.ca/home/%2Flearning-and-development%2Flearning-modules%2F) and the NextGenU Health Assessment Resources Certification (https://courses.nextgenu.org/course/view.php?id=271#section-0) Additional Required Qualifications:

Minimum 2 years of clinical experience working as an RN/RPN preferably in a home care, supportive living or long-term care environment. Knowledge of home care, continuing care and community resources. Demonstrated excellence in written and verbal communication. The successful candidate must have the ability to work in a fast-paced environment. Must be highly self-motivated, adaptable, flexible and able to be a team player. The ability to work in multiple computer programs while completing phone assessments. Strong working knowledge of Meditech, Connect Care, Microsoft Office products. Preferred Qualifications:

Experience with Inter-Rai Contact Assessments. Case management training is preferred.
