The Registered Nurse – Discharge Coordinator’s key responsibility is to proactively facilitate client transitions across the continuum by identifying and considering unmet and discharge needs from the time of a client’s admission to hospital. Working collaboratively with the client, family and care providers with a person-centered approach, the Discharge Coordinator assists the client to plan their return home or to an appropriate Living Option in a safe and timely manner. The Discharge Coordinator practices client and system case management by engaging in case management activities focused on the discharge process, including screening, assessment, and service needs determination and transition planning. In addition to a competitive rate of pay, AHS currently has a Remote Retention Allowance in place to an annual maximum of $3,000. This allowance is non-pensionable and is payable on an hourly basis for all hours paid at the basic rate of pay.

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS.

Required Qualifications:
Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College and Association of Registered Nurses of Alberta (CARNA). Current Basic Cardiac Life Support – Health Care Provider (BCLS-HCP). Additional Required Qualifications:
Minimum 5 years of nursing experience. Demonstrated skills in problem solving, case management, interpersonal relationships, and organizational leadership. Excellent ability in the organization and delivery of care to Seniors Health clients. Ability to work in a fast-paced environment; ability to be flexible to time commitments and to meet the needs of clients in the Seniors Health program. Preferred Qualifications:
Baccalaureate degree in nursing preferred; however, skills and experience relevant to nursing practice in the community sector will be considered. Home Care, Seniors Health, Discharge Planning and Placement or related experience is preferred. Experience in a rural setting will be considered an asset. Knowledge of RAI assessment tools is considered an asset. Completion of the AHS Case Management Education is considered an asset. Excellent ability in case management and coordination an asset.
