What you do

  • Carry out components of established psychosocial and physical rehabilitative programs to address the assessed needs of patients by implementing, actively participating in and evaluating therapeutic programs for assigned areas. Implement individual program plans and corresponding psychosocial and physical rehabilitation intervention strategies including: exercise, programs for specific conditions (i.e. orthopaedic, neurological etc.), ambulation, crutch walking, range of motion (ROM), self-care training, back health groups, the application of specific therapeutic modalities such as Interferential current (IFC), TENS, etc. and a wide variety of group-based rehabilitation and/or vocational rehab programs/services;
  • Work collaboratively with the inter-professional team to develop/implement individualized rehabilitative treatment plans for patients by identifying patient strengths, limitations and functional level for program involvement. Work with team members in the regular evaluation of assigned programs with regard to patients’ assessed needs and progress, and modifies or changes programs as indicated, e.g., recommending suitability for programs. Utilize skills and knowledge to assess a patient’s condition and make in the moment adjustments to interventions in response to the patient’s current health status.
  • Complete rehabilitation assignments within designated scope of practice and documents patient progress. Report observations of patients’ behavior and progress to therapist and Treatment Team. Communicate concerns and/or changes to patients’ condition. Prepare individual discharge reports to meet department/organization documentation standards at the conclusion of each closed program by summarizing information focusing on patients’ life skills and independence levels and recommending appropriate community support where available. Maintains outcome and workload measurement statistics.
  • Work collaboratively with other Rehabilitation colleague, therapists and Treatment Team members to analyze the needs of target patient populations as a basis for new program development, standards and evaluation strategies by reviewing relevant literature and suggesting interventions that will support the achievement of desired treatment outcomes. Participate in the development of specific program curricula to address the functional level of the target population including researching, obtaining, and creating appropriate educational, testing, and other program materials to meet established program goals and objectives and evaluation processes under the guidance and supervision of the designated therapist.
  • Maintain work area and equipment by ensuring equipment and supplies, such as tools, materials and chemicals used in the program are stored appropriately and accounted for, arranging repairs as required, and assuming the responsibility for the purchase of supplies to support programs as required. Clean, disinfect and maintain equipment including mobility self-care aids and adaptive devices, and report malfunctioning equipment to appropriate staff. Fabricate/repair adaptive devices such as splint straps under the direction of the designated therapist.
  • Perform regular security functions to maintain the safety and security of patients, staff, families and the public at all times and takes immediate action to mitigate risks, as well as reporting all real and potential risk issues. Ensure safety and security of the staff, patients and public during all planned activities such as Staff Supported Community Outings.
  • Drive residents to designated locations as scheduled, by operating the Centre’s passenger van/bus with a seating capacity of 11 to 25 passengers, including the driver. Assists clients with embarking and disembarking the vehicle. Conduct inspections of vehicle by checking oil, gas and fluid levels, tire condition and ensuring that lights and signals are functioning; cleans interior and exterior of vehicle after use; and maintains records of distances traveled, gas expenditures and inspections. Recommend vehicle and equipment repairs as required.

What you bring

  • Completion of a Rehabilitation Assistant Program or an equivalent combination of recent, related education and training plus 2 years clinical experience.
  • Food Safe Certificate
  • Valid Class 4 BC Driver’s License.
