This is accomplished across four geographic management zones which are responsible for the operation of acute care health centres and the provision of a variety of inpatient, outpatient services including academic, tertiary, quaternary care and community based programs and services including continuing care, primary health care, public health, and mental health and addictions.

Responsibilities

Reporting to the Health Program Manager, the Continuing Care Coordinator is accountable for case management of clients referred to Continuing Care. This role is inclusive of intake, assessments, identification of unmet needs, development of care plans, authorization of service resources, evaluation, and discharge.

This Care Coordinator position will float between hospital and community care. The responsibilities of the Continuing Care Coordinator in the Community involve drawing on knowledge of health and social services to identify goals and a service plan that address the unmet needs of clients in the complexities of the health care environment. This will involve assessment, problem solving, developing, coordinating and monitoring client’s progress.
The responsibilities of the Continuing Care Coordinator in Hospital include client assessments and facilitation of client discharge from hospital, as well as participation in education, promotion & development activities.

The Continuing Care Coordinator works within teams and in partnership with health care professionals, service providers, acute & long term care facilities, government departments and community agencies to support the Home First Philosophy.

Qualifications

*Bachelor’s degree in Nursing, Nutritional Studies, Occupational Therapy, Physiotherapy or Social Work with 3 – 5 years of work experience within the profession
*Post experience of two (2) years in a community environment focused on sustaining and enabling clients to live independently in the community (equivalents as determined by the employer may be considered)
*Membership in good standing with a professional regulatory governing body in Nova Scotia
*Competent with computer use, keyboarding and be able to conduct assessments using a laptop
*Possess in depth knowledge of community agencies and resources
*Excellent interpersonal, communication, decision-making and assessment skills
*Ability to work independently and co-operatively in a busy, multidisciplinary situation
*Ability to multitask, prioritize workload and manage time efficiently
*Flexibility and willingness to take on new assignments
*Must have full access to a reliable vehicle and a valid driver’s license
*Competencies in other languages preferred; French an asset

Hours of Work

  • Permanent Full-time Position; 70 hours bi-weekly

Salary Information

$38.5066 – $43.8409 hourly
