LMC has 12 multi-disciplinary centers of excellence located in 3 provinces (Ontario, Alberta, and Quebec). Our 60+ Endocrinologists, many of whom are nationally renowned for their areas of expertise, are supported by an interdisciplinary team of highly qualified professionals.
Currently, our team is looking for a Health Coach who can work with us on a temporary part-time basis for 12 months (hours will be based on the needs of the program). This is a remote opportunity.
LMC Diabetes Education Team, in collaboration with the Diabetes Leadership Foundation and Diabetes Canada, is delivering the Type 1 Diabetes Coaching Program. The program is a unique 6-month health coaching program designed to move patients along their individual self-management journey.
Responsibilities
- The health coach complements the diabetes educator (CDE) role; instead of providing patients with education, the health coach will help patients turn the program modules into real-life actions and learnings.
- The health coach will take the time to understand each patient and their own type 1 diabetes experience.
- The health coach will provide patients with a judgement-free space to help them achieve their health goals and help build confidence to overcome any obstacles or challenges life may bring while managing type 1 diabetes.
- Ultimately, the health coach will need to:
- Understand and appreciate that every patient comes with their own experience living with type 1 diabetes.
- Help move patients along their self-management journey.
- Work with patients to set health goals.
- Hold patients accountable for achieving the goals they set out to do – discuss barriers/challenges, collaborate, and problem-solve together.
- Help patients apply the tools & content they’ve learned in order to improve their self-care.
- Push patients to ensure they are completing goals, engaging in self-awareness and making self-care changes to improve their own management.
- Assist with program awareness, screening and recruitment initiatives.
- Assist with administrative tasks as needed.
- Stay current and aware of opportunities to implement new, evidenced-based methods of patient assessment and treatment.
Requirements and Skills
- A Bachelor’s degree in exercise science, nutrition, life sciences, nursing or other health-related field.
- Experience working with individuals with Type 1 diabetes is required.
- Certified Diabetes Educator (CDE) certification is preferred.
- Additional training in health coaching is preferred.
- Experience teaching/counselling individuals and small groups.
- Compassionate.
- Self-motivated.
- Excellent computer skills.
- Excellent communication skills.
- Own a personal computer with high-speed internet access (for working remotely).
